Therapy Requests – you may request access to therapy over the phone or in person or via this website. The request process involves providing your name, address, telephone number and email address. There may also be a request for information on your availability, therapeutic issues, and other details which you deem relevant for processing your request.

Initial Assessment Appointments – at an initial appointment you will be asked about your current personal, social, medical and financial circumstances. You may also be asked about your background and family history, as well as the issues which are affecting you now.

Therapy – if undertaken, a range of personal data will be collected from and about you, including special category (such as health) data.

This website is hosted securely on therapyspace.co. Online enquiries are forwarded directly to Caroline Cosgrove Therapy and are not stored by therapyspace.co. By completing an online enquiry form, Caroline Cosgrove Therapy will hold personal data (your name, email address, phone number and other messages sent). To request deletion of your data, please email carolinecosgrovetherapy@gmail.com.

Confidentiality – Keeping your information safe.

All information disclosed by the client is held in the strictest confidence. Any notes made about sessions are anonymous and destined? 7 years after the last contact session. The only circumstances in which information might be disclosed would be if there were serious concerns about the client's safety or the safety of others. Where possible this would always be discussed with the client first.

Your Rights

You have the right to ask for a copy of the information that held about you. You also have the right to require the correction of any inaccuracies in your information or to request deletion of your data. You may withdraw your consent for your data to be held and processed at any time. However, if you do this while actively receiving therapy, the therapy would have to end. Changes to this policy
